On Wednesday, Entrada Therapeutics, Inc. (NASDAQ: TRDA) reported topline results from the first cohort of its Phase 1/2 ELEVATE-45-201 clinical trial.
The study evaluates ENTR-601-45 in ambulatory patients who possess a Duchenne muscular dystrophy gene mutation suitable for exon 45 skipping.
Following the announcement, the micro-cap biotech stock fell more than 15%.
Future Clinical Milestones And Dosing
Given the specific age of the participants, their baseline functional levels, and the short three-dose regimen, investigators neither expected nor observed any functional benefits at this early stage.
Functional impact will be assessed with higher doses and over longer dosing periods via future cohorts and the open-label studies.
These future trial phases will utilize higher drug concentrations over extended dosing periods, testing doses of 10 mg/kg (data expected in the first half of 2027) in the second cohort and up to 15 mg/kg in the third cohort.
Evaluating Safety And Tolerability Endpoints
Participants in the first cohort received three 5 mg/kg doses of the drug or a placebo. The topline findings met the primary objectives, demonstrating a highly favorable safety and tolerability profile for the treatment.
Headaches ranked as the most frequent treatment-emergent adverse event, occurring five times across two trial participants.
While one patient experienced two serious treatment-emergent adverse events involving vomiting and a fever, these issues subsequently resolved.
The clinical trial recorded no participant discontinuations for any reason. All enrolled individuals progressed to the open-label Phase 2 segment of the study, where they will receive six additional doses of the medication at the 5 mg/kg level.
Furthermore, researchers noted no hypomagnesemia or renal safety concerns during the study. Renal markers, which included eGFR and cystatin C, stayed well within normal ranges for the treated participants. Magnesium levels also remained strictly within the normal parameters.
Biomarker Trends And Early Efficacy Signals
The secondary trial objectives aligned with corporate expectations. Patients taking the active drug exhibited a 0.10% mean increase in dystrophin production compared to baseline levels.
In contrast, the placebo group experienced a 0.02% decrease. Dystrophin measurements utilized western blot analysis normalized to myosin heavy chain.
Additionally, exon skipping increased by a mean of 0.15% from baseline in treated participants, whereas the placebo group recorded a 0.04% decline.
Plasma exposures matched the early predictions derived from juvenile non-human primate data.
Modeled plasma exposures give researchers significant confidence that higher doses in upcoming cohorts will meaningfully increase dystrophin expression.
TRDA Price Action: Entrada Therapeutics shares were down 15.23% at $4.119 at the time of publication on Wednesday, according to market data.
